PHP Form

I have a form in which the user fills out a questionaire of about 30 questions. In each question on has to fill out a value between 1 and five.. while at the same time selecting one of two radio buttons. At the end of the form I want the user to see the result (the final amount) of the 30 different values on the screen. While also sending a email with the info to a email address. Can someone point me in the right direction of a code that I might start off with to accomplish this!
